Healthcare Georgia Foundation has contracted with Georgia Health Decisions to implement a Community Coaches Program to support The Two Georgias Initiative.  The purpose of the Coaches Program is to maximize the success of the community partnerships funded by the Initiative with the following overall goals:

  • Developing strategic approaches

  • Aligning community resources

  • Building robust, adaptive collaborations

  • Structuring initiatives for long-term sustained impact

The Community Coaches will serve as strategic thought partners for the Community Health Partnerships funded in Phase I of The Two Georgias Initiative.              

Each grantee partnership will be assigned a Community Coach who will have an on-going relationship with the community throughout the funding cycle: pre-application, grant award, and continuation funding. Through regular communication and site visits, the Coaches will serve as strategic thought partners, identifying the need for external expertise, facilitating peer learning among The Two Georgias Initiative communities, assisting in leadership development and communication planning, and providing facilitation and capacity building support, as needed.
