Healthcare Georgia Foundation is pleased to announce the Notification of Funding Availability (NOFA) for the 2019 Direct Services Grant Program. In alignment with the Foundation’s new Strategic Plan which addresses achieving health equity for all, the Direct Services Grant Program focuses on Preventing and Managing Chronic Diseases that is included in our Promoting Health and Preventing Disease impact area.
The 2019 Direct Services Grant Program will support new or existing best practice or promising approaches programs that address high burden and costly chronic diseases and underlying risk factors that have proven evidence-based interventions, including: Asthma; Cardiovascular Disease (Heart Disease and Stroke), and Diabetes.
Several Healthcare Georgia Foundation staff members participated in a Direct Services Pre-Application Webinar on Wednesday, July 10th. The webinar, which can be viewed here on demand, covered the goals of this funding opportunity, provided information on new evaluation expectations, and addressed several applicant questions.
