Save the Date: Healthcare Georgia Foundation will announce the Notification of Funding Availability (NOFA) for the 2021 Direct Services Grant Program on February 8, 2021. In accordance with the Foundation’s strategic plan, the Direct Services Grant Program focuses on Preventing and Managing Chronic Diseases. According to the Centers for Disease Control and Prevention, chronic diseases are the leading causes of death and disability in the United States and are also the leading driver of healthcare costs. The Direct Services Grant Program will support programs that address some of the most common and costly chronic diseases that have proven evidence-based interventions, including:
- Cardiovascular disease (Heart disease and Stroke)
People with underlying chronic conditions are more at-risk for contracting and dying from COVID-19. Therefore, it will be essential that potential applicants address how they are helping people to better manage their chronic condition(s) during the pandemic, as well as acknowledge how health equity is being addressed. More details about the NOFA, including timelines, application materials and requirements, will be available on the Foundation’s website at that time.
