Working together to build a healthier Cook County by improving health equity, access, and outcomes.



Community Health Improvement Goals
- Expand the knowledge, skills, and abilities of collaborative partners to serve as empowered leaders in the successful implementation of the Cook County Community Health Improvement Plan (CHIP).
- Strengthen leadership abilities and increase leadership roles/positions assumed by collaborative partners to promote and support implementation of the CHIP.
- Expand and strengthen the diverse human, social, and material capacity of the collaborative partnership so that all assets and resources are maximized.
- Promote the relevance of the Cook County Family Connection partnership and the CHIP implementation to the broader community.
