REQUEST FOR PROPOSALS 

   The Two Georgias Initiative: Phase I Evaluation

Healthcare Georgia Foundation is seeking proposals to design an evaluation of the Foundation’s Two Georgias Initiative (2017-2022) Phase I. The selected evaluator will design and conduct the Initiative-level evaluation, provide evaluation technical assistance to Foundation Community Health Partnership grantees, facilitate peer learning capacity-building opportunities for grantees, and report on results at the end of the grant period.

ONLINE APPLICATION DEADLINE:  FEBRUARY 3, 2017 - 3 pm

This is a multi-level, multi-phase evaluation where the level of engagement by the evaluator needs to be considered for partnerships, organizations, and programs, both collectively and individually, with respect to flexibility and choice. This Request for Proposals (RFP) provides background on the grant program, the scope of work for the evaluation, and the format and deadlines for applications under this Evaluation RFP.
