Session Description:
In this session, we will discuss practical approaches for the primary care practitioner for long-term successful management of patients with Type 2 Diabetes.
Session Objectives:
At the end of this session, participants will be able to:
- Understand the current broad conundrum of managing Type 2 Diabetes in the primary care setting
- Prioritize patient health goals and targets, based on existing literature
- Understand practical strategies for effectively educating patients in the primary care setting
- Understand the practical realities of medication effectiveness
- Utilize basic motivational interviewing skills to successfully enable patient self-care

Participants registering for programs should have access to the following technology for best viewing and participation:
