Session Description:
This session will provide a comprehensive review of the challenges with using medications in seniors. Participants will learn effective strategies to ensure seniors can actively participate in managing their medications.

Session Objectives:
Session participants will learn how to:

  1. Understand causes and impact of Adverse Drug Events
  2. Describe Medication Reconciliation, when and how to perform
  3. Be able to use available evidence-based resources to ensure that seniors' care reflects safe prescribing of medications
