Community Coaches
Healthcare Georgia Foundation has partnered with Georgia Health Decisions to provide Community Coaches as strategic thought partners to the 11 rural Community Health Coalitions funded through The Two Georgias Initiative as they implement plans to reduce rural health disparities in their communities. The purpose of the Coaches Program is to maximize the success of the community partnerships funded by the Initiative with the following overall goals:
- Developing strategic approaches
- Aligning community resources
- Building robust, adaptive collaborations
- Structuring partnerships for long-term sustained impact

Community Health Partnerships as they
move forward with their local health planning.

Each Two Georgias grantee has been assigned a Community Coach who will has an on-going relationship with the coalition throughout the funding cycle: pre-application, grant award, and continuation funding. Through regular communication and site visits, the Coaches serve as strategic thought partners, identifying the need for external expertise, facilitating peer learning, assisting in leadership development, communication planning, and providing facilitation and capacity building support as needed.
